Please bring a completed 5-page Patient Intake Form, to your first appointment. If you are unable to download and print the PDF below, please come to your appointment 20 minutes early to do so. This will allow us to use your time more effectively. Thank you.

Cancellation Policy

In order to continue to offer high quality care at affordable prices, I ask for 24 hours notice in advance of an appointment if you need to cancel or reschedule.

All appointments that are rescheduled or cancelled with less than 24 hours advance notice, and appointments missed without notice, will be charged the regular fee for that appointment.  If appointments have been purchased in a package, the missed, cancelled or rescheduled appointment will be deducted from the number of remaining appointments in that package. Thank you for your understanding.
